SECOND - AUDITION FORM - supplies information about the auditioner,
as well as gives parents a chance to indicate the volunteer committee they would like to serve on (all families must serve on volunteer committees)

FINALLY -  CONFLICT SHEET - please write your child's conflicts.   If the Artistic Team casts your child, the listed conflicts are excused.   Additional absences (not listed on this form) will affect the production numbers that your child performs in.
